RECOGNIZING CRASH SURVIVORS WHO BUCKLED UP
The "Saved By The Belt" Award is a statewide program to honor Utah drivers and passengers whose decision to buckle up prevented serious injury or death in a motor vehicle crash. The program is administered by the Utah Department of Public Safety’s Highway Safety Office (UHSO) and helps spread the message that seat belt use is important to individuals and communities across the state. Regardless of the circumstances surrounding a motor vehicle crash, a person is more likely to survive when they are protected by a seat belt or child car seat.
Award recipients will receive a certificate of recognition, a vehicle license plate cover and choose between a T-shirt or auto nylon vehicle sunshade. To qualify, please complete the online application, which will provide the UHSO with important information concerning you and the crash. One form must be completed for each individual who was “Saved by the Belt.” If you prefer to mail or fax your application, download a PDF copy of the application, complete it and send it to the address/number listed on the form. An internal review committee will evaluate the application and determine if the applicant is eligible for a “Saved by the Belt” award. We appreciate your willingness to share your story. SAVED BY THE BELT WALLET CARDS
These wallet-sized cards are for use by law enforcement, EMS and other emergency or hospital personnel when they encounter a crash survivor who was probably saved by their seat belt, child car seat or booster seat. These cards provide direction on how the survivor can access the “Saved by the Belt” program information. To obtain these cards, you can either download and print or contact the UHSO at 801-366-6040 to order wallet cards for distribution to survivors.
